We report a case of a male patient with multiple gastro-duodenal-jejunal angiodysplasias, previously treated with argon plasma coagulation (APC) through double-balloon enteroscopy (DBE) advanced up to 200 cm. Due to recurrent bleeding, an anterograde DBE was performed using Endorail®. Angiodysplasias distal to the first tattoo were treated with APC, exploring up to 330 cm of the small bowel. The Endorail® system is recently approved for enteroscopy with CE marking. It uses a magnetic balloon that can be advanced beyond the enteroscope tip, filled with a ferromagnetic fluid, and magnetically anchored to the patient’s abdomen, allowing it to be straightened and guided along the device. Once the challenging bowel segment is negotiated, the balloon guide can be retrieved. The Endorail® system appears to enable deeper exploration of the small bowel in difficult cases.
