Successful Complete Closure Of Anastomotic Leak In Ileal Pouch Anal Anastomosis With Endoscopic Through-the-scope X-TACK Helix Suturing System

Anastomotic leaks remain a serious complication after ileal pouch–anal anastomosis.We report the case of a 53-year-old male patient who underwent laparoscopic subtotal colectomy with ileal pouch for ulcerative colitis. On the fifth postoperative day, the patient developed fever and purulent output from the surgical drain and computed tomography demonstrated an anastomotic leak. The patient was referred to the gastroenterology department for endoscopic management. During endoscopy, a leak approximately 20 mm in diameter was identified near the dentate line and was completely closed using the X-Tack Helix suturing system. Follow-up endoscopy at four and twelve months after the procedure demonstrated the X-Tack system in place. The patient showed clinical improvement soon after the endoscopic closure and remained asymptomatic through the 12-month follow-up.
