Role of bedside glucometry in differentiating mucinous vs non mucinous cystic lesions of pancreas

Aims
To study the sensitivity,specificity and accuracy of bedside glucometry in diagnosing nature of cystic lesions of pancreas.
Methods
Study was conducted in Medanta,The Medicity hospital Gurgaon,India and total number of patients were 56 and all patients with age more than 18 years and with pancreatic cystic lesions were included.
EUS was done in each patient and FNA was performed and pancreatic cystic fluid was collected immediately and glucose was checked using commercial glucometer ie Accu Check.
Results
Glucose level less than 60 mg/dl correctly differentiated mucinous from non mucinous pancreatic cystic lesions among 42.9% patients with sensitivity and specificity of 82.8% and 81.5% respectively.The area under curve was high (0.88,p=0001).The positive predictive value,negative predictive value and accuracy was 82.8%,81.5% and 82.1% respectively.The mean CEA level was significantly (p=0.0001) higher among patients with mucinous (4844.5+/- 90.24) compared to non mucinous (262.9+/- 366.9).
CEA level more than 100 correctly differentiate mucinous from non mucinous cysts with sensitivity and specificity of 82.8% and 70.4% respectively.The positive predictive values,negative predictive value and accuracy was 75%,79.2% and 76.8% respectively.
