Real-Time Artificial Intelligence enhanced Cholangioscopy

A 90-year-old woman with an indeterminate hilar biliary stricture presented with jaundice, weight loss, and abdominal pain. MRCP showed irregular intrahepatic and common hepatic duct dilation suggesting Bismuth type IV stricture. AI-enhanced cholangioscopy enabled live estimation of neoplastic probability also identifying aberrant vessels supporting malignancy. Dynamic probability shifts helped define tumor extent, while overlapping heat maps suggested optimal biopsy sites. Targeted biopsies confirmed cholangiocarcinoma. When validated, AI may aid in differentiating biliary stenosis, defining tumor extent, and improving biopsy accuracy.
