A rare cause of an upper gastrointestinal obstruction – Bouveret’s syndrome in 73-old man

Bouveret’s syndrome is a gastric outlet obstruction caused by gallstone blockage. We report a case of a 73-years old man who was admitted to the hospital with a symptoms of upper gastrointestinal (GI) obstruction. In CT scan Ringler’s triad was observed, including intestinal obstruction, pneumobilia, and an aberrant gallstone in the bowel. At admission we decided to perform endoscopic removal of the gallstone from the duodenum, which after many attempts with a loop and an extraction basket was unsuccessful. Subsequently, the surgical treatment with omega loop gastric bypass was accomplished. In the postoperative period patient developed upper GI obstruction. Due to persistent symptoms and increase in inflammatory parameters relaparotomy was conducted upon which the right hepatic lobe abscess was revealed and removed.  In the following days patient presented intolerance of oral alimentation therefore gastroscopy with insertion of nasointestinal feeding tube was performed. To supplement the calorie deficit and adjust the diet to the metabolic demand the patient was referred to a nutritional treatment center.
