Rendu–Osler–Weber syndrome is characterized by fragile arteriovenous malformations that can lead to recurrent upper gastrointestinal bleeding. In this case, initial endoscopic therapy using standard haemoclips achieved only temporary control, with two subsequent episodes of rebleeding from an oesophageal AVM. During the third bleeding event, bipolar coagulation was insufficient to obtain haemostasis. After removal of previously placed haemoclips, an over-the-scope clip (OTSC) was deployed directly onto the malformation, providing stable mechanical haemostasis. Minor residual mucosal oozing from the top of the tissue grasped by the clip was successfully treated with a bipolar probe. Following this combined intervention, no further bleeding occurred. To our knowledge, this is the first reported use of an OTSC for haemostasis of an oesophageal AVM.
