This video shows 2 cases of patients with extended multifocal low-grade dysplasia in the antrum with proximal extension. The lesions were removed en-block via ESD with the method of 2 wings butterfly. This technique involves the creation of two submucosal flaps bilaterally while preserving a central submucosal septum between the two flaps, creating a structure which resembles the body of a butterfly between its wings. The flaps are created in the anterior and posterior wall of the stomach, while gravity reveals the central submucosal septum. The incision is then advanced from the distal to the proximal end of the lesion. Aim of this method is to use the gravitational effect in order to accelerate the dissection of large gastric lesions.
