Per oral endoscopic myotomy (POEM) is currently the first-line treatment for achalasia. Myotomy is the step most prone to complications, namely bleeding or electrocoagulation organ injury, and its technique is still variable around expert centers. We report a case of type 1 achalasia with a 10-point Eckardt score, treated with laser-assisted POEM. After standard esophageal mucosal incision and submucosal tunnel creation with a TriangleTipKnife J (Olympus, Tokyo, Japan) a 9-cm selective myotomy was performed using a 365 μm holmium laser fiber (Quanta System, Samarate, Italy). Patient resumed oral liquid diet on day 1 and was discharged after 24 hours. At 3-month follow-up patient achieved clinical remission and weight regain. Holmium laser fiber technology offers an easy, safe and precise myotomy method during POEM.
