LAMS dislodgement during EDGE: not all about anchoring

We present a case of a 36-year-old woman post gastric bypass who underwent EDGE for suspected CBD stones. After LAMS placement, the tract was left to mature for four weeks. During the subsequent ERCP, a perpendicular axis of the endoscope to the axis of a distally placed LAMS was encountered. Further attempts at scope advancement, despite severe friction, resulted in LAMS dislodgement. The absence of wire introduction made endoscopic salvage impossible. Intraoperative ERCP during emergency surgery ultimately revealed no stones. This case highlights the importance of factors other than LAMS anchoring in preventing stent dislodgement.
