EUS-Guided Embolization of a Pancreatic Pseudoaneurysm

Visceral artery pseudoaneurysms are rare but serious complications of chronic pancreatitis, often presenting with upper gastrointestinal bleeding (UGIB).A 68-year-old man with alcohol-related chronic pancreatitis and liver disease presented with recurrent UGIB.Initial endoscopic and radiologic studies were unremarkable.During a new episode, CT angiography showed a 20 mm lesion near the main pancreatic duct consistent with a pseudoaneurysm.Angiographic embolization failed due to non-identification of the lesion.Using a linear echoendoscope with Doppler from the distal duodenum, the pseudoaneurysm was located in the pancreatic uncinate process.EUS-guided thrombin injection (500 IU) achieved complete Doppler resolution.CT at 24 hours confirmed thrombosis, and the patient remained clinically stable.EUS-guided thrombin embolization is a safe, effective alternative when angiographic treatment is not feasible.
