Endoscopic Removal of Migrated Mesh Sutures 26 Years After Restrictive Gastroplasty

Restrictive gastroplasty is now obsolete because of long-term complications, including late staple-line and foreign-body problems (1,2). A 52-year-old woman who underwent restrictive gastroplasty 26 years ago presented with recurrent vomiting and abdominal discomfort for months. Upper endoscopy revealed severe gastritis, marked esophageal and distal gastric pouch dilatation, and intraluminally migrated mesh sutures at the anastomotic site. We demonstrate stepwise endoscopic release and retrieval of three migrated mesh sutures using electrocautery and endoscopic scissors, without immediate complications. The patient was monitored for 24 hours without adverse events and remained asymptomatic at six-week follow-up on proton pump inhibitor and dietary counselling. Our case highlights endoscopic mesh removal as a minimally invasive alternative to revisional surgery (3).
