A 61-year-old woman, was found to have an incidental gastric subepithelial tumour (SET) on upper-GI endoscopy performed for anemia. EUS revealed a fusiform-shaped SET, measuring 8.2x5.7-mm, hypoechoic pattern, arising from the muscularis propria. Using a multi-band mucosectomy device, successful SET ligation was performed without resection. Deep tissue sampling was achieved via the SINK technique. Histopathology (DOG1+,CD117+,actin+) confirmed spindle-cell GIST. Serial EUS follow-up at 6-weeks, 12-months, 2 and 5 years demonstrated no residual or recurrent tumor. The Banding-SET technique combined with SINK for GISTs ≤10mm is feasible and safe offering an alternative therapeutic management for small SETs with malignant potential, with a low recurrence risk over 5-year follow-up ( BANDING-SET study: ClinicalTrials.gov NCT03247231).
