Endorail-Assisted PEG in absent abdominal transillumination: A Case Report

We describe the case of a 45-year-old woman with a neurogenic syndrome of the interstitial cells of Cajal and a history of multiple abdominal surgeries. An initial attempt at PEG placement failed due to the inability to obtain an adequate abdominal transillumination window. A CT scan revealed a complete intrathoracic stomach, so to address this issue, we decided to use a bowel magnetic traction system. An out-of-scope ferromagnetic balloon catheter was positioned in the stomach, and through an external magnet, controlled traction was applied to the organ towards the epigastrium. This enabled the creation of a safe and stable transillumination window, allowing for a gastropexy with anchoring devices and the subsequent completion of the PEG placement via the "pull" technique, without complications.
