EDEE failed despite successful through-the-LAMS limb access in a post-Whipple patient with relapsing cholangitis and a complex benign HJS without intrahepatic dilatation. Direct jejunal EUS-guided needle puncture from the efferent jejunum allowed target distention and freehand 20-mm LAMS placement into the afferent limb. After jejuno-jejunal LAMS-balloon dilation a gastroscope was advanced to the HJS. The surgical anastomosis could not be found. A linear EUS scope was then carefully passed through the LAMS. The left hepatic duct was imaged and punctured under combined EUS & fluoroscopy. Cholangiography confirmed bilateral HJS stricture with sclerosing cholangitis. A plastic stent was placed into the LHD from the afferent jejunum. Post-procedure bleeding was managed endoscopically and biliary drainage re-established.
