The Clip with a Grip: Taming Wide ESD Defects in the Stomach

Closing mucosal defects after endoscopic submucosal dissection (ESD) reduces delayed bleeding, but large gastric defects are difficult close with conventional clips that lack sufficient opening width and traction. The Dual-Action Tissue (DAT) clip, with two independently controlled arms, enables sequential edge capture and controlled approximation before reinforcement with standard clips. We report two gastric ESD cases in which the DAT clips achieved effective closure. In Case 1 (32×31 mm specimen) a single 15-mm DAT clip plus three conventional clips secured the defect. In Case 2 (31×30 mm specimen, fibrotic) one DAT clip plus four standard clips achieved full closure. Deployment was easy, with no device-related adverse events. Both patients recovered uneventfully. The DAT clip appears valuable for closure of large ESD defects.
