Cholangioscopy-guided multimodal treatment of a complex postsurgical biliary stricture with embedded suture materal

A 65-year-old woman with right hepatectomy for hydatidosis and history of ERCPs in other center for biliary stricture was referred to our hospital. Cholangiography revealed a stricture at the hepatic hilum with a 30 mm-dilatation of intrahepatic duct. An attempt to cannulate the intrahepatic ducts with a guidewire and sphincterotome was unsuccessful. Cholangioscopy identified the stricture with an impacted stone. Electrohydraulic lithotripsy was carried out, after which a bundle of suture material was visualized. Using SpyBite, part of the suture was grasped and removed. Subsequently, guidewire passage across the stricture was achieved. The stricture was dilated with a 6-mm Hurricane balloon, and a 10 Fr × 15 cm straight plastic stent was placed. Post-surgical biliary strictures represent a diagnostic and therapeutic challenge. Cholangioscopy is very useful for the diagnosis and management of complex biliary strictures.
