Gastrointestinal Stromal Tumors (GISTs) is a form of rare mesenchymal tumors and accounts for < 1% of all gastrointestinal tumors. GISTs are rarer in the colon. A 44-year-old male had irregular bowel movements, rectal bleeding and right iliac fossa pain. Colonoscopy revealed an ileum mass intussusception into the ascending colon. Histology revealed gastric tissue lined by hyperplastic epithelium, proliferating mesenchymal neoplasm composed of atypical epithelioid cells disposed basically in sheets with central nuclei and moderate eosinophilic to clear cytoplasm. > Ten mitoses in 40hpf and focal congestion, necrosis, and hemorrhages seen. Features suggestive of Epitheloid GIST. A hemicolectomy was performed with an oncology review: The patients one year post surgery surveillance colonoscopy was normal. This case underscores the role of colonoscopy in diagnosing rare tumors.
