Breaking Through Stent Overgrowth: A Step-by-Step Endoscopic Solution

Palliative treatment of malignant esophageal strictures with self-expanding metal stents (SEMS) can be complicated by tissue overgrowth, making retrieval difficult and risky.We present a case of a patient with esophagogastric junction cancer and a double SEMS (stent-in-stent), who developed severe dysphagia due to stent overgrowth.A combined approach was performed: radial incisions with a needle knife on the proximal overgrowth to expose and free the inner stent edge, which was then removed with rat-tooth forceps.For the outer stent, with its distal flange deeply embedded in the gastric wall, dissection using an ESD knife released the flange, allowing safe stent removal.The patient experienced immediate symptomatic relief.This combination of endoscopic techniques enabled safe retrieval of deeply embedded stents, avoiding more invasive procedures.
